Could the clock on the wall play a bigger role in cancer treatment than previously thought?

Well, emerging research suggests that the timing of cancer therapies may significantly influence how well patients respond, opening the door to a promising field known as cancer chronotherapy.

Scientists are increasingly discovering that the body's natural circadian rhythm – the internal 24-hour biological clock that regulates sleep, hormone production, metabolism and immune function - may also affect how cancer cells and healthy tissues respond to treatment.

The findings could eventually help doctors personalise treatment schedules to improve outcomes while reducing unwanted side effects.

What did the study say?

The study published in Nature Medicine early 2026 examined 210 patients with advanced non-small cell lung cancer who were randomly assigned to receive first-line immunochemotherapy either before 3:00 pm or after 3:00 pm.

Researchers reported that patients treated earlier in the day experienced longer overall survival and progression-free survival than those who received treatment later.

The findings suggested that the body's circadian rhythm may influence how effectively the immune system responds to cancer therapy, although the researchers stressed that further studies would be needed to confirm the results.

Cancer chronotherapy is the practice of administering treatments such as immunotherapy, chemotherapy or radiotherapy at specific times of the day to align with a patient's biological clock.

Rather than changing the medicine itself, chronotherapy focuses on when treatment is delivered. Researchers believe this simple adjustment could enhance the body's ability to fight cancer while limiting damage to healthy cells.

The concept has existed for decades, but advances in circadian biology and cancer research are providing stronger evidence that treatment timing genuinely matters.

Morning immunotherapy may improve survival

One of the latest studies has found that patients receiving cancer immunotherapy earlier in the day experienced significantly better outcomes than those treated later.

Researchers reported that morning treatment was associated with a substantially lower risk of death, with some patients seeing up to a 63 per cent reduction in mortality risk compared with afternoon recipients. Recent research examining radiotherapy found that tumour cells may become more vulnerable to radiation at certain times of day because their ability to repair DNA damage changes over the circadian cycle.

Researchers identified the circadian protein CRY1 as an important regulator of DNA repair. When CRY1 levels are naturally higher later in the day, tumour cells appear less capable of repairing radiation-induced DNA damage, potentially making radiotherapy more effective for some patients.

These discoveries could eventually help oncologists determine the optimal treatment window for individual patients.

Traditional cancer therapies often damage healthy cells alongside cancer cells, leading to fatigue, nausea, infections and other complications.

Chronotherapy represents an exciting step towards more personalised cancer care. Instead of relying solely on new medicines, clinicians may eventually improve outcomes simply by optimising the timing of existing treatments.

If ongoing clinical trials continue to produce positive results, hospitals could one day schedule chemotherapy, immunotherapy and radiotherapy according to each patient's biological clock, potentially increasing survival rates while reducing treatment-related complications.

Although more research is required before widespread adoption, the evidence suggests that something as simple as the time on the appointment schedule could become an important factor in the future of cancer treatment.
