Why fungal infections remain a major threat even after a successful bone marrow transplant
A successful bone marrow transplant marks a new beginning, but a weakened immune system leaves patients vulnerable to dangerous fungal infections requiring strict prevention and early intervention.

A successful bone marrow transplant (BMT) is often seen as a life-changing milestone for patients battling blood cancers, aplastic anaemia, thalassaemia and immune disorders. However, doctors warn that the end of the transplant procedure does not mark the end of the medical journey.
The months following transplantation remain one of the most vulnerable phases as the patient’s immune system takes time to rebuild. During this period, fungal infections continue to be among the most serious complications, capable of affecting the lungs, bloodstream and other organs if not diagnosed and treated quickly.
Unlike common infections, fungal infections after a transplant can progress silently in the beginning. Mild symptoms such as persistent fever, fatigue or cough may appear similar to routine post-transplant recovery issues, making early diagnosis challenging.

Dr. Rahul Bhargava, Principal Director and Chief of Hematology, Hemato-Oncology and Bone Marrow Transplant at Fortis Gurgaon, said that a successful bone marrow transplant is a major milestone for patients being treated for blood cancers, aplastic anaemia, thalassaemia, and certain immune disorders. However, recovery does not end after the procedure. The post-transplant period requires careful monitoring as the immune system remains weakened for weeks or months, making fungal infections one of the most serious concerns, he said.
Before transplantation, patients undergo intensive treatments to remove diseased cells and prepare the body for healthy stem cells. While this improves the chances of recovery, it temporarily weakens the immune system, allowing fungi commonly found in the environment to cause severe infections, He added.
Studies show that invasive fungal infections affect a significant number of patients undergoing allogeneic bone marrow transplants and continue to contribute to high mortality rates, especially when diagnosis and treatment are delayed.
Why fungal infections remain a concern
Dr Bhargava: Several factors increase the risk following transplantation:
• Prolonged suppression of the immune system
• Low white blood cell counts during recovery
• Use of steroids and immunosuppressive medicines
• Graft versus host disease requiring additional treatment
The importance of prevention
Dr Bhargava: Preventive care is a key part of post-transplant management. High-risk patients are often given antifungal medicines, while infection control measures, specialised transplant units, and regular follow-ups help improve outcomes.
Important preventive measures include:
- Timely use of prescribed antifungal medicines
- Regular monitoring and follow-up assessments
- Immediate medical evaluation when symptoms appear
- Avoiding exposure to dust, mould, and construction environments
Early detection remains critical, as advances in diagnostic tests and imaging allow doctors to identify infections sooner and begin treatment promptly.
Although fungal infections remain a serious concern after bone marrow transplantation, increased awareness, preventive strategies, and timely medical care have significantly improved survival outcomes. Continuous monitoring during immune recovery remains essential for ensuring safe and successful long-term recovery.
Early symptoms can be difficult to recognise
One of the biggest challenges in managing fungal infections is that early warning signs are often non-specific.
Dr. Ajay Sharma, Director and Head of Hematology and Hemato-Oncology at Paras Health (Panchkula) said symptoms can sometimes be mistaken for normal post-transplant complications.
“Early symptoms can be mild or unclear, leading to delays in diagnosis. Timely identification and quick action are critical to reducing the severity of illness and protecting long-term transplant outcomes,” he said.
Doctors advise patients and families not to ignore warning signs such as:
Persistent fever
*Difficulty in breathing
*Chest pain
*Continuous cough
*Unusual weakness
*Sudden decline in overall health
Early investigations, including imaging and advanced diagnostic tests, can help doctors identify infections sooner and begin appropriate treatment.
Prevention remains the strongest defence
Experts emphasise that preventing fungal infections is as important as treating them. Dr. Suparno Chakrabarti, Principal Director – Transplantation and Cellular Therapy (AIBTraCT), Action Cancer Hospital (Delhi) said post-transplant care requires strict discipline because patients may appear healthier externally while their immunity remains fragile internally.
“Recovery after transplant is not only about improvement in blood counts but also about protecting patients during the immune rebuilding phase,” he said.
Preventive measures recommended by doctors include:
Taking prescribed antifungal medicines regularly
Maintaining strict personal hygiene
Avoiding dust, mould and construction areas
Following food safety precautions
Wearing masks when advised
Avoiding crowded places during high-risk periods
Attending regular follow-up appointments
Specialised transplant units, infection-control protocols and preventive antifungal strategies have significantly improved patient outcomes. However, doctors stress that even small lapses in precautions can create serious risks during immune recovery.

Doctors say awareness among patients and caregivers is crucial because the emotional relief after a successful transplant can sometimes lead families to underestimate ongoing risks.
A transplant may restore blood production, but complete immune recovery takes time. Maintaining vigilance during this phase can determine whether a medical success becomes a long-term recovery.
