Magnesium supplements: Benefits, side effects and who should avoid them
Magnesium is having a moment in wellness circles, credited with everything from better sleep to fewer muscle cramps. Here's what the research actually supports, and who should check with a doctor before adding it to their routine.

Magnesium is a cofactor in more than 300 enzyme systems in the body, involved in energy production, DNA synthesis, muscle contraction, nerve signalling, blood sugar control and blood pressure regulation. Despite that reach, a large share of adults fall short of getting enough from food alone, which is a big part of why magnesium supplements have become a fixture of wellness routines and sleep stacks.
The appeal rests on that reach across bodily systems. Magnesium helps relax muscles after exercise, supports steady heart rhythm, and plays a role in how the nervous system manages stress, which is why so many people take it before bed. A significant share of the body's magnesium is stored in bone, so adequate intake is also linked to better bone density over time.
It intersects with two of the most searched health concerns of recent years: sleep and anxiety. Magnesium helps regulate neurotransmitters that calm the nervous system, and some research points to modest improvements in sleep quality and stress resilience with supplementation. The effect tends to be more meaningful in people who were actually deficient to begin with, rather than a dramatic upgrade for someone already getting enough. For migraines, muscle cramps and premenstrual symptoms, magnesium is often recommended clinically and anecdotally, though the evidence here is more mixed than for its foundational roles in bone and heart health.
Not all forms are equal
Magnesium oxide, one of the cheapest and most common forms on pharmacy shelves, is poorly absorbed and has a stronger laxative effect than other forms, which is exactly why it's often used in over-the-counter constipation remedies. Forms like magnesium citrate, glycinate, lactate and chloride dissolve more readily in liquid and are absorbed more efficiently, making them the better choice when the goal is correcting a deficiency or supporting sleep rather than the bowels. Magnesium glycinate in particular has a reputation for being gentler on the stomach, since it's bound to an amino acid rather than pulling water into the intestine the way other forms do.
The side effects
The most common downside is gastrointestinal: nausea, cramping and diarrhoea, especially at higher doses or with less bioavailable forms like magnesium oxide. This happens because magnesium draws water into the intestines, an effect mild enough to be a feature in laxatives and a nuisance when it isn't intended. Starting with a lower dose, taking it with food and increasing gradually usually helps.
At the extreme end, magnesium toxicity is rare in people with healthy kidneys but becomes a real risk when kidney function is impaired, since the kidneys are what clear excess magnesium from the blood. Signs of overload include very low blood pressure, irregular heart rhythm, muscle weakness and, in severe cases, breathing difficulty — almost always tied to supplement overuse or poor kidney clearance rather than diet.
Magnesium also interacts with a handful of common medications. It reduces the absorption of certain antibiotics, including tetracyclines and quinolones like ciprofloxacin, as well as osteoporosis drugs like bisphosphonates, so doctors typically advise spacing doses two to four hours apart. It can also amplify the effects of diuretics and some heart medications.
Who should be cautious
People with kidney disease need to be most careful, since impaired kidneys struggle to excrete excess magnesium. Certain heart conditions call for similar caution, given magnesium's effect on cardiac rhythm and its interactions with heart medications. People with myasthenia gravis, an autoimmune condition affecting muscle strength, should also be wary, since high doses can worsen muscle weakness. Anyone on diuretics, antibiotics or bisphosphonates should check timing and dosage with a doctor first.
Older adults sit in a trickier spot: they're more prone to both deficiency and, if kidney function has declined with age, toxicity, which makes medical guidance especially useful rather than optional. Pregnant or breastfeeding people generally tolerate magnesium well and need slightly more of it, but should still take supplements under medical guidance rather than self-prescribing high doses.
For most healthy adults, a moderate dose of a well-absorbed form like citrate or glycinate is unlikely to cause harm and may close a real dietary gap. For anyone with kidney issues, heart conditions or a regular medication routine, it's worth a conversation with a doctor first — magnesium's biggest risks tend to show up exactly where its interactions are least visible.
