Can insulin resistance make it harder to conceive? Doctors explain
Insulin resistance may quietly reduce fertility in both men and women, even before diabetes develops. Fertility experts explain the warning signs, risks and ways to improve reproductive health.

For many couples struggling to conceive, the focus often falls on reproductive health alone. But fertility specialists say an underlying metabolic condition, insulin resistance, may be quietly reducing the chances of pregnancy, even in people who have normal blood sugar levels.
Often associated with type 2 diabetes, insulin resistance begins years before diabetes develops. During this phase, the body's cells become less responsive to insulin, prompting the pancreas to produce more of the hormone to keep blood sugar in check. Over time, these persistently high insulin levels can disrupt the hormonal balance needed for healthy ovulation, sperm production and successful pregnancy.
The condition is increasingly being recognised as an important, yet frequently overlooked, contributor to infertility among young Indians.
What is insulin resistance?
Insulin is a hormone that helps move glucose from the bloodstream into the body's cells, where it is used for energy.
When cells stop responding effectively to insulin, the body compensates by producing more of it. While blood sugar levels may remain normal initially, elevated insulin levels can affect multiple organs, including the ovaries and testes.
"Insulin resistance is showing up more as a kind of overlooked contributor to infertility, especially among young Indians. People usually tie it to diabetes, but it starts way before that, and it keeps reaching into more than just blood sugar control," says Dr Ajantha Boopathi, Fertility Specialist at Nova IVF Fertility, Chennai.
How does insulin resistance affect female fertility?
One of the strongest links is with Polycystic Ovary Syndrome (PCOS), now increasingly referred to in some clinical settings as Polycystic Ovarian Morphology Syndrome (PMOS). Women with insulin resistance often produce excess insulin, which stimulates the ovaries to make higher levels of male hormones, or androgens.
These hormonal changes can interfere with ovulation, cause irregular menstrual cycles and reduce egg quality.
"Higher insulin nudges the ovaries toward making extra androgens. Those hormones can affect ovulation, throw off menstrual regularity and even lower the overall egg quality," says Dr Boopathi.
She adds that the impact isn't limited to women diagnosed with PCOS. "Even women who do not have PCOS can still see reduced fertility if insulin resistance is left alone and not properly addressed."
During assisted reproductive treatments such as IVF, uncontrolled insulin resistance has also been linked to lower implantation rates, a greater risk of miscarriage and pregnancy-related complications.
Men aren't immune either
Fertility specialists emphasise that insulin resistance affects both partners.
In men, it is associated with obesity, chronic inflammation, hormonal imbalance and oxidative stress—all of which can compromise sperm quality.
"It may impair sperm count, movement and DNA integrity. So, infertility should be looked at as a couple's problem, not only one partner's, because it is rarely just one side," Dr Boopathi explains.
Why is it often missed?
One of the biggest challenges is that insulin resistance rarely causes obvious symptoms in its early stages. A person may have perfectly normal fasting blood sugar while underlying metabolic dysfunction continues to progress. According to Dr Boopathi, some warning signs include:
-Increasing abdominal fat
-Darkening of skin folds (known as acanthosis nigricans)
-Irregular menstrual cycles
-A family history of diabetes
-Difficulty losing weight despite lifestyle changes
These clues should prompt further metabolic evaluation rather than waiting until diabetes develops.
Lifestyle may be making things worse
Experts say changing eating habits and increasingly sedentary lifestyles are contributing to the problem.
According to Dr Priyanka Reddy, Consultant – Infertility Specialist at Motherhood Fertility and IVF, Bengaluru, many people mistakenly believe avoiding sweets alone is enough to protect their metabolic health.
"People often think that if they avoid sweets, they are safe, while ignoring the impact of refined carbohydrates," she says.
Foods such as white rice-based preparations, refined flour products and other rapidly digested carbohydrates can also trigger sharp rises in blood sugar if consumed without enough protein, fibre or healthy fats.
She explains, "The tendency of people is to believe that if they just refrain from eating sweets, then they are in a safe zone while ignoring the threats involved in consuming refined carbohydrates. Foods such as poha, idli, and dosa contain a high amount of carbohydrates and consumption of these foods leads to quick rise in blood sugar level. Carbohydrates need to be balanced with other kinds of food like protein and fats."
She also points to lifestyle habits that can worsen insulin resistance, including:
-Poor sleep
-Late-night eating
-Prolonged screen time
-Chronic stress
-Sedentary work routines
"The inability to sleep well, late-night eating, excess computer usage and increased stress lead to metabolic malfunctioning," she says.
Can insulin resistance be reversed?
The good news is that insulin resistance can often be improved, and in some cases significantly reversed, with early intervention. Dr Boopathi recommends:
-Achieving and maintaining a healthy weight
-Regular physical activity
-Eating more fibre and protein while reducing refined carbohydrates
-Prioritising good-quality sleep
-Managing stress through activities such as reading, hobbies or journalling
-Taking medication when recommended by a doctor
"Acting early can improve fertility outcomes, and it also lowers the long-term risk of diabetes, cardiovascular disease and pregnancy-related complications," she says.
Fertility begins before pregnancy
Doctors say conversations around fertility often begin only when couples struggle to conceive. However, experts argue that metabolic health deserves equal attention during preconception planning.
A simple evaluation for insulin resistance, particularly in people with irregular periods, obesity, a family history of diabetes or unexplained infertility, could help identify a reversible factor before it affects reproductive outcomes.
