From hysterectomy to hope? How robotic surgery is preserving fertility in complex uterine conditions
Experts say timely diagnosis and advances in minimally invasive gynecological surgery are enabling more women with fibroids and endometriosis to explore uterus-preserving treatment options.

Advanced minimally invasive technologies are shifting the paradigm from traditional hysterectomies to precise, uterus-preserving procedures, offering new hope to women balancing medical treatment with reproductive goals.
For generations, women facing severe benign uterine conditions like massive fibroids or advanced endometriosis were often left with a devastating, binary choice: live with debilitating pain and bleeding, or undergo a hysterectomy (the complete surgical removal of the uterus). The latter, while curative, permanently ends any hope of biological childbearing.
However, a quiet revolution is taking place in modern operating rooms. Driven by breakthroughs in medical technology, robotic-assisted surgery is redefining gynecological care. By offering unprecedented precision, 3D visualization, and enhanced dexterity, these advanced platforms are allowing surgeons to successfully treat complex conditions while carefully reconstructing and preserving healthy uterine tissue, giving women a crucial lifeline to protect their future fertility.
The cost of silence: The danger of normalising symptoms
A major hurdle in women’s reproductive health is the tendency to downplay severe symptoms. Many women endure years of agony before seeking medical help, often dismissing warning signs as "just a bad period."
Doctors say that advances in minimally invasive and robotic-assisted gynecological surgery are now transforming treatment pathways, especially for women hoping to preserve fertility while addressing complex uterine conditions.
Dr. Meenakshi Sundaram, Senior Consultant and Robotic Surgeon – Obstetrics and Gynaecology at Apollo Women’s Hospitals (Chennai) said many women tend to tolerate symptoms for years before seeking medical attention.
“Many benign uterine conditions such as fibroids or endometriosis often begin with symptoms that women gradually learn to live with rather than investigate. Heavy bleeding, pelvic discomfort, fatigue, or painful periods are frequently dismissed as routine menstrual concerns, even when they begin affecting sleep, work, or daily activities,” she said.
She stressed that menstrual pain severe enough to disrupt everyday life should not be ignored.
“Studies suggest that only 25% of women may experience symptoms associated with fibroids, and in my clinical practice, nearly one in four women visiting the clinic present with fibroids or related symptoms,” Sundaram noted, adding that delayed medical intervention can reduce treatment choices as the disease advances.
Shifting from open surgery to high-tech precision
When conditions like endometriosis or fibroids are left unchecked, they can wreak havoc on the female reproductive system. By the time a patient finally sees a specialist, the disease has often progressed significantly, resulting in enlarged uteri, dense tissue adhesions, and severely distorted pelvic anatomy.
Historically, these advanced cases required large-incision open surgeries, which carry longer recovery times and higher risks of scarring, or a total hysterectomy.
Dr. Anshumala Shukla Kulkarni, Head – Minimally Invasive Gynaecology, Gynaecology Laparoscopic and Robotic Surgery at Kokilaben Dhirubhai Ambani Hospital, Mumbai, said endometriosis in particular often goes undiagnosed for years.
“Studies have shown that endometriosis may take several years to be diagnosed because symptoms such as severe menstrual pain, pelvic discomfort, and heavy bleeding are often normalised or overlooked,” she said.
By the time many women consult specialists, she explained, the disease may already have progressed substantially, resulting in enlarged uteri, dense adhesions, and distorted pelvic anatomy that make surgery far more challenging.
“Traditionally, many advanced cases required open surgery and, in some situations, hysterectomy. Today, advances in minimally invasive gynecological surgery, including robotic-assisted platforms such as the da Vinci Surgical System, are enabling surgeons to perform complex procedures with enhanced visualisation, precision, and instrument dexterity,” Kulkarni said.
She added that in carefully selected patients, robotic-assisted approaches may support uterus-preserving procedures such as myomectomy, in which fibroids are removed while reconstructing healthy uterine tissue wherever clinically feasible.
Similarly, robotic-assisted surgery is increasingly being explored in complex endometriosis cases where disease tissue may involve anatomically difficult areas of the pelvis.
Doctors warn, however, that robotic surgery is not universally suitable and treatment decisions depend on several factors, including the severity of disease, age, symptoms, reproductive goals, and overall clinical evaluation.
Beyond benign uterine conditions, fertility preservation is also becoming part of broader conversations in gynecological care, including selected cancer cases.
Expanding the horizons: Balancing disease management and fertility
The conversation around preserving reproductive organs is expanding even beyond benign conditions. In highly specific cases, it is entering the realm of early-stage oncological care.
Dr. Dimple K Ahluwalia, Associate Director, Gynaecology and Gynae Oncology at Medanta Hospital (Gurugram) said advances in minimally invasive and robotic-assisted procedures are helping expand fertility-preserving possibilities in appropriately selected patients.
“In certain early-stage gynecological malignancies, fertility-preserving treatment strategies may be considered after careful clinical evaluation, depending on factors such as the type and stage of disease, the patient’s age, overall health, and future reproductive plans,” she said.
According to Ahluwalia, robotic-assisted procedures may support precise surgical dissection and uterus-preserving approaches in selected cases while maintaining focus on oncological safety.
“An important aspect of modern gynecological care is balancing effective disease management with preservation of reproductive potential wherever clinically feasible,” she added.
Experts collectively emphasised that early diagnosis remains central to improving outcomes. Timely consultation with specialists can help women better understand the full range of treatment options available including minimally invasive and fertility-preserving procedures that may not be feasible in more advanced stages of disease.
With growing awareness and evolving surgical technologies, doctors say the focus in gynecological care is increasingly shifting from simply treating disease to preserving long-term reproductive health and quality of life wherever medically possible.
